How much of what we see when visiting the Iowa today was there when the ship was launched? I realize something like this will never be in a Ship of Theseus situation, but some things had to have been replaced over the decades. Is 90 to 95% of it still original?

Most of the living spaces have been refitted fairly heavily in the 80s. Most of the spaces open to visitors were originally there when the ship was built, although the aft superstructure is new. But that's mostly not on the tour route. And there's obviously stuff like the Tomahawk ABLs. It probably depends on how you measure it.
